New Orthopedic Surgeon Joins Madison County Health Care System

Madison County Health Care System (MCHCS) is pleased to announce that orthopedic surgeon Dr. Teri Formanek, MD has joined the organization and will begin seeing patients on August 1. Dr. Formanek will be performing general orthopedic procedures and specializes in upper extremity procedures, having completed the Harvard Hand/Upper Extremity Fellowship. He will see patients at Health Trust Physicians Clinic in Winterset four days per week and at the Adair County Health System (ACHS) in Greenfield one day per week.

“We are thrilled Dr. Formanek will be joining our team,” said MCHCS & ACHS CEO Marcia Hendricks. “His expertise and experience are outstanding and we’re confident he will provide the best possible care to our patients.”

Dr. Formanek completed his medical degree at the University of Iowa and his orthopedic surgery residency at the Creighton-Nebraska Foundation in Omaha. He is certified by the American Board of Orthopaedic Surgery with additional qualifications in hand surgery. He is an active member of the medical community with professional affiliations with the American Academy of Orthopaedic Surgeons, American Society for Surgery of the Hand, American Medical Association, and Iowa Medical Society. His work has been published in Foot & Ankle International and the Nebraska Medical Journal.

“I decided to come to Madison and Adair County Hospitals for the great opportunity the hospitals had to offer,” said Dr. Formanek. “This has been a dream of mine for several years.  I have always practiced in large metro areas and this is a chance to get back to my roots as a person who grew up in rural Iowa.”
