The RN Health Coach is responsible for providing telephonic, electronic, face to face, and group visits for disease management/case management. Performs other clinically based activities to coordinate care between the members, providers and community based health organizations. Uses Evidence Based guidelines and methods in practice.
RN license in good standing. BSN a plus but not required. Health Coach Certification (Will provide training if needed)
