What is a Patient-Centered Medical Home?

Both Health Trust Physicians Clinic and Earlham Medical Clinic operate as a Patient-Centered Medical Home (PCMH). A PCMH is not a specific building, but rather an approach to providing comprehensive health care.

It is called a medical “home” because it should be the first place you think of for all your health care needs. Our goal is to make it easy and comfortable to get the care you need in a way that works best for you.

As your health care provider, we are here to facilitate a personal partnership with you and your family to provide you with high quality, comprehensive, and evidence-based health care.

Our Responsibilities as Your Trusted Health Care Team:

  • Provide a safe and healthy environment to receive care
  • Partner with you in making health care decisions
  • Coordinate with you, your authorized representatives, and other health care providers
  • Keep you informed and on-track

Your Responsibilities as a Patient:

  • Be an active team player
    • Talk with your team about your health questions
    • Share your past health care successes and challenges
    • Tell your team about other health care professionals who care for you
    • Tell your team how you feel about the care you are receiving from them
  • Take care of your health
    • Follow the health care plan you and your team have talked about
    • Make sure you understand how to follow the plan
    • Set goals you can reach
    • Once you begin to see results, you and your team can discuss adding new goals
  • Talk openly with your team
    • Tell your team if you are having trouble sticking to your care plan
    • Work with your care team to make changes if needed

Health Coaches

Our health coaches provide education and support to patients by helping them develop customized plans to help them reach their personal health goals. This may include education on specific conditions or risk factors, healthy eating tips, developing a realistic exercise plan, and setting up follow-up visits.

Health coaches work with people with chronic diseases such as diabetes, heart disease or high blood pressure, or with people who are heading in a direction that could lead to a more serious health situation.

Your provider may suggest you visit with one of our health coaches, or you may schedule an appointment on your own by calling the Health Trust Physicians Clinic at 515-462-2950.

Health Coach services are available at no charge for Health Trust Physicians Clinic patients.

Patient Portal

The HealtheLife Patient Portal provides secure, online access to your personal medical information. The portal allows a patient to send a secure electronic message to their healthcare team, request an appointment, view visit summaries with vital signs, lab results, and some other diagnostic test results. A caregiver can also view a patient’s medical record (authorization required).
