| Billing Information and Important Numbers |
Billing Information As a courtesy, we will file your insurace claim, but you are ultimately responsible for paying for the services provided. During registration, you will be asked to sign a Registration form. This formm allows us to care for you as a patient, file a claim with your insurance carrier and have your carrier make payments directly to Madison County Health Care System. Inpatients will be asked to sign additional Admission paperwork. Medicare recipients will be asked questions to determine if Medicare is the primary or secondary payer. Financial Payments Madison County Health Care System request the patient portion of charges be paid at the time of registration. The amount you pay varies depending on your insurance coverage. We request co-payment and deductible amounts be paid at the time of service. Remaining balances such as coinsurance will be billed after the insurance company processes the claim. Madison County Health Care System accepts the following forms of payment: personal checks, cash, Master Card, Discover , American Express & Visa. If payment at time of service is not possible, Madison County Health Care System will perform services that are indicated to be medically necessary. Pre-Certification Many insurance plans have pre-certification requirements. Generally, patients or their physicians are responsible for obtaining pre-certification. In order to ensure that a claim gets preauthorized for payment, it is rquired that patients present ALL insurance information at the time of registration. If complete insurance information is not provided prior to treatment, the patient's guarantor may be liable for the entire bill. To avoid penalties or reduction of payments, call your insurance carrier(s) for specific information.
Medicare Madison County Health Care System will bill medicare on your behalf for services rendered. You are responsible for any applicable deductibles, co-insurance or other amounts not paid by Medicare, including but not limited to, self administered drugs in outpatient settings such as Emergency room and observation. For your convenience, your supplemental carrier will be billed for any balances following Medicare's payment.
Medicaid We require all Medicaid patients to present a current eligibility card at the time of registration. Without verification of coverage, you will be responsible for your account. We will bill Medicaid when all necessary information has been provided. you are responsible for non-covered portions associated with your individual coverage.
Commercial Insurance We will bill your insurance carrier if you have provided us with all necessary policy information, including a copy of your insurance card. In the absence of necessary billing information, you will be responsible for payment of your entire account. In the event your insurance carrier is not prompt in making payment, madison County Health Care System may need your assistance in contacting the carrier for payment. In cases where the service has not yet been paid, you will be personally responsible for the bill.
Worker's Compensation Your employer must approve Worker's Compensation coverage prior to your registration. Madison County Health Care System will not become involved in disputes between you and your employer other than to supply factual information. If your claim is denied by your employer r their worker's compensation insurance, you are ultimately liable for the care you received.
Third Party Liability If you have been injured in some manner, whether automobile accident, fall, etc., another party may have some legal responsibility for your injuries. Although this is a matter between you, your attorney, and the party responsible for the injury, Madison County Health Care System will bill on your behalf to the carrier you provide. If you are a Medicare or Medicaid recipient, health care laws require that we bill the liaility carrier first, then Medicare or Medicaid. Financial Arrangements
Madison County Health Care System has staff available to discuss your individual financial needs. If you do not have inurance or will have difficulty in paying your portion of the bill, the staff can assist you in making payment arrangements or may suggest alternate financial assistane options. Patients will receive necessary medical care, and the care will never be based on the patient's ability to pay.
