Patient receives specialized nursing and medical care.

| Acute Care |
|
Patient is monitored and treated for conditions of acute illness.

| Skilled or Swing Bed Care |
|
Patient status can
be changed from acute care to skilled. This allows for a period of
rehabilitation or continued treatment. Physician sees patient no less
than weekly.

| Hospice Care |
| Patient is
either being treated to control acute symptoms or may be in for
palliative or comfort care only. Hospice nurses and physician manage
care with family and patient input. |
