Chronic Disease Management

Diabetes Education Program Merits ADA Recognition

Madison County Memorial Hospital’s Diabetes Education Program has been awarded continued recognition from the American Diabetes Association.  The program was originally recognized in September of 1999.  Assuring high-quality education for patient self-care is one of the primary goals of the Education Recognition program.  Through the support of the health care team and increased knowledge and awareness of diabetes, the patient can assume a major part of the responsibility for his/her diabetes management.
The Diabetes Education & Management Program is designed to increase patient access to diabetes education in order to promote optimal health and prevent the high costs and complications of this disease.

We work in partnership with specialty physicians, primary care physicians, physician’s assistants and nurse practitioners to help the person with diabetes learn the content and skills necessary for successful self-management. Certified
diabetes educators (CDE), both a registered nurse and registered dietitians, who utilize the case management and empowerment models in the delivery of care are on staff. Recognized by the American Diabetes Association in 2000, and recertified in 2006, the ADA standards of care and self-management education provide the framework for the program's curriculum.
