Diabetes Education Program Merits ADA Recognition
Madison County Memorial Hospital’s
Diabetes Education Program has been awarded continued recognition from
the American Diabetes Association. The program was originally
recognized in September of 1999. Assuring high-quality education for
patient self-care is one of the primary goals of the Education
Recognition program. Through the support of the health care team and
increased knowledge and awareness of diabetes, the patient can assume a
major part of the responsibility for his/her diabetes management. |
The Diabetes Education & Management Program is designed to increase patient access to diabetes education in order to promote optimal health and prevent the high costs and complications of this disease.
We work in partnership with specialty physicians, primary care physicians, physician’s assistants and nurse practitioners to help the person with diabetes learn the content and skills necessary for successful self-management. Certified diabetes educators (CDE), both a registered nurse and registered dietitians, who utilize the case management and empowerment models in the delivery of care are on staff. Recognized by the American Diabetes Association in 2000, and recertified in 2006, the ADA standards of care and self-management education provide the framework for the program's curriculum.

| Cardio/Pulmonary Rehabilitation |
|
Patients are referred for rehabilitation after an acute cardiac episode such as Myocardial Infarction (heart attack), open heart surgery or chronic lung disease. Care is supervised by certified nursing staff who specialize in cardiac services.

Cardiopulmonary Testing
|
|
Outpatient options are available to monitor the heart beat for 24 hours to 30 days to help diagnose heart rythm problems. Exercise tolerance tests are available to help diagnose a problem with blood flow to the heart muscle.
| Anticoagulation/Coumadin Clinic |
|
The Anticoagulation/Coumadin Clinic at Madison County Memorial Hospital assists patients in the management of anticoagulation therapy, including warfarin (Coumadin®) therapy.

Our services include:

Easy access and a simplified registration process

Testing by the most accurate laboratory method

Immediate test results/dosage adjustment

Ongoing patient education

Clinician consultation

Monitored medication management
-
Ongoing communication with the patient’s physician
A Health Trust Physician’s Clinic physician referral is required for participation in the Clinic. Upon receiving a referral authorization, patients will be scheduled for an initial visit.The Clinic is held in the hospital lab, on Tuesday mornings and other days as needed.
Anticoagulation/Coumadin Clinic Staff

Becky Paxton, RN, CDE has been operating the Anticoagulation Clinic since 2001. She is trained in anticoagulation management and also sees patients for other chronic conditions such as diabetes, heart disease and pulmonary disease. |
