Billing Information And Important Numbers
At the time of registration you will be asked to provide a current insurance card. Uninsured patients will be asked to provide a current drivers license. Also, any specific forms or original documentation (claim forms, authorizations, referrals, etc.) required by your insurance company are needed. As a courtesy, we will file your insurance claim, but you are ultimately responsible for paying for the services provided. During registration, you will be asked to sign a Registration Form. This form allows us to care for you as a patient, file a claim with your insurance carrier and have your carrier make payments directly to Madison County Health Care System. Inpatients will be asked to sign additional Admission paperwork. Medicare recipients will be asked questions to determine if Medicare is the primary or secondary payer.
Financial PaymentsMadison County Health Care System requests the patient portion of charges be paid at the time of registration. The amount you pay varies depending on your insurance coverage. We request co-payment and deductible amounts be paid at time of service. Remaining balances such as coinsurance will be billed after the insurance company processes the claim. Madison County Health Care System accepts the following forms of payment: personal checks, cash, Master Card Discover & Visa. If payment at time of service is not possible, Madison County Health Care System will perform services that are indicated to be medically necessary.
Pre-CertificationMany insurance plans have pre-certification requirements. Generally, patients or their physicians are responsible for obtaining pre-certification. In order to ensure that a claim gets preauthorized for payment, it is required that patients present ALL Insurance information at the time of Registration. If complete Insurance information is not provided prior to treatment, the patient’s guarantor may be liable for the entire bill. To avoid penalties or reduction of payments, call your insurance carrier(s) for specific information.
Billing StatementsNormally, the insurance carrier will send an explanation of benefits explaining what they have paid and what the patient’s portion is to pay. Once the balance due is the patient’s responsibility to pay, Madison County Health Care System will send a statement every 28 days to each patient. We find it is less confusing for the patients if they each receive a monthly statement. If you are unable to pay the balance in full, please call 515-462-9747 to set up a reasonable payment arrangement. If timely payments are not received, your account may be forwarded to a collection agency or a judgment may be filed in Small Claims Court.
Insurance InformationInsurance companies do not always pay for all the medical services you receive. You will be responsible for paying for any service not covered by your insurance policy. Due to the large number of insurance plans, patients are responsible to understand their individual insurance policies and preferred provider listings. Most insurance companies routinely audit some of their claims. If your insurance company selects your claim, you may experience a delay in your claim being paid. Madison County Health Care System works with your insurance company to provide any detailed information needed to complete their audit, but we have no control on the length of time it takes to complete the process. If your insurance company requests additional information from you, please respond promptly to avoid denial of payment.
MedicareMadison County Health Care System will bill Medicare on your behalf for services rendered. You are responsible for any applicable deductibles, co-insurance or other amounts not paid by Medicare. For your convenience, your supplemental carrier will be billed for any balances following Medicare’s payment.
MedicaidWe require all Medicaid patients to present a current eligibility card at the time of registration. Without verification of coverage, you will be responsible for your account. We will bill Medicaid when all necessary information has been provided. You are responsible for non-covered portions associated with your individual coverage.
Commercial InsuranceWe will bill your insurance carrier if you have provided us with all necessary policy information, including a copy of your insurance card. In the absence of necessary billing information, you will be responsible for payment of your entire account. In the event your insurance carrier is not prompt in making payment, Madison County Health Care System may need your assistance in contacting the carrier for payment. In cases where the service has not been paid, you will be personally responsible for the bill.
Workers CompensationYour employer must approve Workers Compensation coverage prior to your registration. Madison County Health Care System will not become involved in disputes between you and your employer other than to supply factual information.
Third Party LiabilityIf you have been injured in some manner, whether automobile accident, fall, etc., another party may have some legal responsibility for your injuries. Although this is a matter between you, your attorney, and the party responsible for the injury, Madison County Health Care System will bill on your behalf to the carrier you provide. If you are a Medicare or Medicaid recipient, health care laws require that we bill the liability carrier first, then Medicare or Medicaid.
Financial ArrangementsMadison County Health Care System has staff available to discuss your individual financial needs. If you do not have insurance or will have difficulty in paying your portion of the bill, the staff can assist you in making payment arrangements or may suggest alternate financial assistance options. Patients will receive necessary medical care, and the care will never be based on the patient’s ability to pay.Financial assistance options:
